Insurance
We participate with most HMO and PPO insurance companies. Using the information you provide us when
you schedule your appointment, we will verify your insurance. If we are unable to verify your insurance, you will be
responsible for payment in full at the time of the visit. Please refer to your insurance handbook for specific benefits and
limitations of your health insurance policy.
Our billing department will file insurance claims to all participating plans. If we are not on your
particular plan, we will provide you with a statement so you may file your claim. It will be your responsibility to file for
secondary insurance. In the course of filing, should there be a dispute with your insurance company, we will attempt to
resolve it for you. However, if your insurance has not paid your claim within 90 days, the balance will be transferred to
you for resolution. Please remember, your insurance policy is a contract between you and the insurance company; therefore,
the balance is your responsibility. Co-payments, co-insurance, and deductibles are due at the time of visit.

Our trained insurance referral coordinators are here as a resource to help you in the referral process. If your plan
requires a referral, you will need to call our office and speak to a coordinator. Our coordinators can also assist you in locating a
doctor who participates in your plan even when a referral is not required. When you leave a message for a coordinator, please leave the
following information:
- Your child's name and birthdate
- The name or type of specialist you need to see (e.g. ENT, cardiologist)
- The time and date of the appointment
- A daytime phone number where you can be reached (home, cell, work)
Please allow 3 working days to complete a referral. Same day referrals are reserved for emergencies only.
We welcome new patients to our practice. You may download informational forms and bring
them to your first visit. Please bring your insurance card and child's immunization records with you. We always ask that any
minor be accompanied to our office by a parent or legal guardian.
