Office Policy:
We welcome new patients to our practice. You may download informational forms and bring
them to your first visit. Please bring your insurance card and child's immunization records with you. We always
ask that any minor be accompanied to our office by a parent or legal guardian.
We understand the need for a child to be seen on an "emergency basis" and will do our best to have you seen promptly.
"Walk-in" emergencies may be Triaged by our trained staff so that we may quickly assess the problem and have your child
seen by a Provider.
Children must be acompanied by custodial parents at all check ups. No vaccine will be administered
unless signed for by a parent. If a guardian brings your child for a sick visit, we must have a signed authorization
on file by the custodial parent for each child brought in for an appointment. Young adults (18 and older) can be seen
without a guardian if there is current consent from a parent.
Copayments, co-insurance, and deductibles are due at the time of visit.
