At each well-check visit, we ask that the parent/guardian/patient update the HIPAA Form or HIPAA Form 18 PLUS . This form provides our office with a list of individuals that are authorized to accompany your child(ren) to our office, discuss your child’s medical care, and assume medical decision making in your absence.

Triage Team
The Triage Team at Snellville Pediatrics helps to determine the level of urgency presented by your child’s illness or condition so that we may better serve all our patients. This team is fully staffed and medically trained to answer the many questions you may have as a parent of a well, sick or injured child from infancy through young adulthood.
