Preparing for Your First Appointment

To prepare for your first appointment, please try to arrive 15 minutes before your scheduled appointment time. When you arrive, you will be asked to complete a few patient forms on a tablet. If for any reason you are unable to keep your appointment, please contact us to cancel or reschedule at least 24 hours in advance of your appointment. We will do everything possible to reschedule your appointment in a timely manner.

Please note: Any minor child must be accompanied to our office by a parent or legal guardian.
